We are a non-profit organization providing dental clinic services, prevention, education and outreach via our Garden Grove clinic, Smile Mobiles, the Smiles X-Press (Teledentistry Program), and CHOC hospital location.

This position will work with our Smiles X-Press (Teledentistry) program providing access to oral health services such as prevention, education, and linkages to dental homes at Orange County schools.

The Care Coordinator will provide information and referrals for pediatric dental services for children ages 20 and under in Orange County. The Care Coordinator will provide excellent customer service to all callers.

Essential Functions:

Smiles Connection OC Referral Line

• Provide high quality and accurate information and referral services to families who contact the Smiles Connection OC referral line.

• Establish and maintain relationships with identified service dental providers

• Tracks and assists parents accessing community partners to receive services for their child.

• Engage the caller (typically the parent/guardian) into a comprehensive dental health system of care.

• Responsible for providing warm and pleasant customer service in assisting families with children to find a dental provider

Teledentistry Care Coordination

• The Care Coordinator will actively engage parents, explain the treatment plan, and link the patient to the most appropriate dental provider.

• Maintain and oversee case load of clients accessing services at the Smiles X-Press sites.

• Follow up in a timely manner with patients who have been identified at the Smiles X-Press sites needing follow up offsite services.

• Work closely with the Teledentistry team to ensure that offsite referral patients complete treatment and assist in mitigating barriers to care.

Position Qualifications:

• High school diploma or GED

• At least one year of dental assisting with front desk experience or treatment counseling

• Able to speak and write English and Spanish proficiently

• Ability to communicate effectively and respectfully with individuals from different racial, ethnic, cultural and social-economic backgrounds.

• Experience working with one or more of the following: families, community groups, nonprofits, health programs,

• Computer literate in Word and Excel programs
